As library professionals, we kept noticing the same pattern. Staff were unaware of incidents, or had no good way to manage them. Branches operated in silos. Paper logs piled up and valuable information was lost. We started with incident management because that's where staff were drowning. The same gaps showed up in room booking, inventory, and facilities, so we kept going.
